What to Expect

Kentucky State Law is clear in their expectation the the patient and medical cannabis provider have a bona fide patient-provider relationship. 
This requires an in-person visit, review of the medical history, discussion on the medical, therapeutic, and palliative properties of medical cannabis, possible risks, benefits, side effects, and potential drug interactions, and planned follow-up.  

Evaluation Process 

SCHEDULING AN EVALUATION APPOINTMENT DOES NOT GUARANTEE A SUCCESSFUL APPLICATION 

YOU MUST MEET THE REQUIREMENTS TO QUALIFY FOR A MEDICINAL CANNABIS CARD. ONCE THE CERTIFICATION IS SIGNED YOU MUST APPLY FOR YOUR CARD ONLINE WITHIN 60 DAYS. IF YOU DO NOT APPLY WITHIN 60 DAYS YOU WILL BE REQUIRED TO COMPLETE ANOTHER EVALUATION APPOINTMENT
