Appointment Types

Initial Evaluation: This visit is required. Medical records must be reviewed and documented before a medical cannabis certification can be completed. You must ALREADY have a formal diagnosis of one of the qualifying conditions verified in your medical chart to receive a certification for medical cannabis. We DO NOT make new diagnoses for patients.  

Qualifying conditions include 

• Any type or form of cancer

• Chronic or severe pain

• Epilepsy or other intractable seizure disorders

• Multiple sclerosis, muscle spasms, or spasticity

• Chronic nausea or cyclical vomiting syndrome

• Post-traumatic stress disorder

Renewal Appointment: Renewal visits are required. The frequency is determined on a case by case basis. Schedule your renewal at your certification visit to ensure your place on the schedule.​
